It takes about four minutes. There are thirteen questions.

Your answers are anonymous. Nothing you answer is stored against your name or your email address.

What we collect

Your answers to the questions. Some of these ask about your health.
The date and time you submitted.

What we do not collect

Anything that identifies you is never stored against your answers.
Your employer, or anything that records who sent you this link.
Your IP address, or any details about your device.

I understand that some of these questions ask about my health, and I agree to my anonymous answers being used for this research.

You can stop at any point before you submit and nothing is saved. Once you submit, your answers are anonymous, so we cannot find your individual response to remove it.
